I hereby authorize and request Body Future Gym to initiate debit orders against my bank account specified in my membership contract, in accordance with the terms and conditions outlined herein, for the purpose of collecting payment for the Body Future Gym Loyalty Programme. I understand and agree that the debit order will be processed monthly for the amount of R35.

1. I am an active member of Body Future Gym and currently hold a valid gym contract with them.

2. I understand that the monthly cost of R35 for the Prestige Membership will be debited from my bank account on a monthly basis.

3. I authorize Body Future Gym to collect payment for the Prestige Membership by debit order, using the bank account details provided in my membership contract.

4. I acknowledge that any changes to the monthly cost of the Prestige Membership will be communicated to me in advance, and I will be given the opportunity to cancel the debit order should I choose not to accept the new cost.

5. I am responsible for ensuring that sufficient funds are available in my bank account to cover the monthly debit order payment.

6. I understand that I may cancel this debit order mandate at any time by providing written notice a month prior to Body Future Gym by sending it eersteriver@bodyfuture.co.za

7. I acknowledge that cancellation of the debit order mandate will not automatically terminate my gym contract with Body Future Gym. Separate cancellation procedures and terms apply for the gym contract.

8. I agree that any disputes or queries related to the debit order transactions should be directed to Body Future Gym in writing to eersteriver@bodyfuture.co.za within 30 days of the transaction date
